We are creating our new 5-year Strategic Plan, which will guide Horizons from 2027 to 2031.

We want to hear from People We Support, Family Members, Natural Supports, Guardians, and Staff. Your ideas and feedback will help us plan for the future.

This survey is anonymous. We do not collect names, email addresses, or IP addresses. Responses will be used to inform our Strategic Plan 2027-2031. Participation is voluntary, and you may stop at any time.

Once all survey responses have been received, we plan to hold focus groups to explore the themes and issues raised in greater detail.
